We Support Virginia Families Navigate CCCPlus Waiver and EPSDT Services
The first step in accessing CCC Plus waiver services is to request a Long-Term Services and Supports (LTSS) screening through your local Department of Social Services. This screening is sometimes referred to as a UAI (Uniform Assessment Instrument) and is used to determine your eligibility for long-term care services.
If you qualify, you will receive a copy of your approved UAI. This assessment allows you to explore several care options, including:
Receiving an approved UAI does not automatically mean you are enrolled in the CCC Plus Waiver. Enrollment occurs only after the required documentation is completed and submitted to your insurance provider by your Service Facilitator. Services cannot be reimbursed until waiver enrollment is officially approved, so it is important to complete this process before care begins.
The next step is scheduling an intake appointment with your Service Facilitator. During your initial intake appointment, your Service Facilitator will work with you to develop a person-centered plan of care tailored to your needs. This plan is submitted to your insurance provider, who will determine the number of care hours authorized each week. Your Service Facilitator will guide you and your caregiver through every step of the process — including completing the necessary paperwork to ensure services are approved and caregivers are paid appropriately.

After your Service Facilitator intake appointment is completed, a referral will be submitted to your insurance provider’s Fiscal Employer Agent (FEA). The Fiscal Employer Agent is responsible for managing employer-related responsibilities such as processing payroll, withholding and depositing taxes, verifying timesheets, and completing required background checks for caregivers.
You will have the option to complete the enrollment process either online or using paper forms. The online option is typically faster and can help prevent delays in starting services.
